A Mystery Attack is a Main Quest in Lost Ark. A Mystery Attack is part of the main story of Lost Ark and it is necessary to complete it in order to complete the game. However, it is recommended to complete some non Main Quests of your choice to gain additional equipment and level up in order to make Main Quests easier by being better equipped and prepared. Completing Main Quests will eventually lead you to unlock new features of the game and gain a full experience of it.

Lost Ark: A Mystery Attack Information & Walkthrough
Once you arrive at the Rethramis Border, look for Merchant Jeda in the area and speak to him. He'll tell you that he's managed to escape safely but unfortunately, Veness didn't. He then tells you that his caravan is just north from where you are now and asks if you could see if it's okay. Now follow the main quest icon presented on your map, it should lead you to where Merchant Veness is.
Once you reach Merchant Veness, he'll immediately scream for help as his wares are being engulfed in flames and asks for help to put out the fires. Now you'll be tasked to protect the carriage and its surroundings. You will need to exterminate the 3 Plague Pests around the carriage and use the water pail to put out the carriage fire. Once you've completed the task, return to Merchant Veness and speak to him. He prays that some of their goods survived the flames. He then says to just follow the road ahead if you're making your way towards the monastery and warns you about the place since almost everyone in that area caught the plague. He says that even the monks are sick which leaves no one to take care of the others.
Now look for Brother Alred in the Monastery, just follow the quest icon presented on your map and it should lead you to the Regria Monastery. Once you reach the area, speak to Brother Alred, a cutscene will then occur. He says that the place is in grave danger, the plague is spreading rapidly and their medicines are useless. Anyone who tries to lend a hand just gets sick. He then asks for your help.
After your conversation with Brother Alred, the quest is completed, and you'll automatically obtain the next main quest A New Disease.
